How long does it take to process my form?

For Patients

Submit Your Form Click the “Submit a Form” button on the home page to get started.
Provide Your Information Fill out the required general information and upload your form securely.
Form Review & Completion Once received, your physician and the VitalMed team will review and complete your form.
Notification & Payment After your physician signs the form, you’ll receive an email notification with payment instructions.
Receive Your Completed Form Once payment is confirmed, your completed form will be sent to you via secure email.

For Physicians

1. Direct your patient to the VitalMedForms website (www.vitalmedforms.ca) to submit their form.

What are the processing times?
